Referral Requirements
A doctor’s referral may be required depending on your insurance. Give us a call if you’re unsure.
Fill out Paperwork
Complete forms in-office or download them to fill out ahead of time and bring with you.
Your First Visit Checklist
Arrive 15 minutes early for your appointment
Bring your Photo I.D. and Insurance Card
Any referral or prescription (if applicable)
Comfortable clothing (if doing physical therapy)
List of past medical history, previous treatments, current medications, and other important information.
Bring a list of questions and concerns you would like to discuss with your therapist.
